The protagonist Anna (played by Milla Jovovich) accidentally witnesses a murder on the street and is chased by the perpetrator after being discovered. In a frantic struggle, Anna is injured and falls into a river, narrowly escaping death. Anna becomes the only survivor who has seen the murderer’s true face. The police decide to find a breakthrough in the serial killer case through Anna. A week later, the injured Anna wakes up from a coma, only to find that every face she sees is constantly changing and morphing — she can no longer recognize the faces of those around her, including her boyfriend and father. This condition is medically known as prosopagnosia, or face blindness. The murderer seems to be aware of Anna’s secret regarding her face blindness due to brain injury. To test whether Anna truly cannot recognize him, the murderer continuously provokes her through various means, making Anna feel the killer is lurking nearby, threatening her life at all times. Eventually, the murderer confirms that Anna can only identify different people based on characteristics like clothing color and tie style. He decides to take a risk by wearing Anna's ex-boyfriend's clothes and tie to confront her, preparing to take her to a secluded place to eliminate her. However, a critical mistake gives Anna a sudden realization that the man before her is not her ex-boyfriend but the very criminal trying to kill her. After Anna alerts the police, they rush to rescue her, leading to a life-and-death struggle, ultimately capturing the murderer.
